Health Forms
Campers cannot come to camp unless we have a current Health Form on file for them. If your Health Form has been misplaced, please download a new copy here and be sure to fill it out and mail or fax it to us before your child arrives at camp. These forms are in Adobe pdf format.

Travel Information and Forms
Farm & Wilderness provides limited Charter Bus service in addition to airport, train and commercial bus pickups. Here we provide two Adobe pdf documents which help those who need to use these arrangements.
The Travel Information Sheet provides information on our travel services, including parameters, costs, and deadlines for signing up.
The Travel Form is the form you will need to fill out if you wish to reserve travel services. We can't make travel reservations on the phone and will need to have this form from you by June 1st for July session and full session campers, and July 1st for August session campers.
